What name will the new hire use? This will be used for their professional display name on all company e-mails, phone calls, Teams messages, and more. Changing this name during the onboarding process can take hours, so please take a few minutes to verify their preferred display name prior to completing this form.
Some companies have different job titles depending on the system or scenario. Please input the title colleagues should see on the new hire's Microsoft 365/Teams profile.

This will be used for Microsoft 365 Multi-Factor Authentication (MFA) security prompts as well as the number CCIT will call or text to coordinate with the new hire and kick-off their IT Onboarding Call on their start date. If the number will be provided at a later date, please let us know in the additional information at the end of this form.
Most full-time employees will require a full GoTo user license, be assigned a full extension, and be listed in the company directory when people call the main line. However, different hires may have different needs and some shouldn't be assigned an extension at all. Please confirm what we need to have in place on Day 1 for a smooth onboarding & training experience. Changes & tweaks can always be made later. If a work phone is not needed, please put N/A.
If the new hire needs to be able to edit PDFs, they can do so using either Foxit PDF Editor or a version of Bluebeam Revu (Core or Complete). Bluebeam is more for those needing to work with construction drawings while Foxit lacks any special Architectural, Engineering, & Construction (AEC) features. NOTE: Presently, only Dennis Anderson has a Bluebeam Revu "Complete" license. All other Bluebeam users have a "Core" license.
Which drive(s) should we ensure the new hire has access to on Day 1?
Assuming this user will need access to Sage, please enter the name of an existing Sage user we should copy the permissions settings of. If Sage is not needed for the new hire, enter "N/A". If custom permissions or special considerations are required, please elaborate.

CCIT can pre-add new hires to certain teams to save you time. Please confirm which teams you'd like visible to the new hire on Day 1. For security purposes, we will NOT add anyone to a group or team without explicit approval.
NOTE: This question is in "beta" testing. Please let us know if any Teams should be added or removed from this IT Onboarding Form.

Describe the name, serial number, and/or location of an existing computer we should configure for the new hire. If possible, please help us ensure it is online so we can immediately access it remotely. Otherwise, let us know any special computer hardware requirements we need to prepare.
NOTE: If a NEW computer is needed, please specify details such as which previous computer to match and/or please call us to confirm pricing, availability, and shipping/prep lead time!

Where will the new hire be seated? Please detail any specific docking station, monitor, keyboard, mouse, and/or headset requirements we need to have ready for Day 1. Is it already setup? If not, are you setting it up or will you need our help onsite? Also, if the new hire will be working from home instead of or in addition to the office, please describe the situation and what (additional) equipment will need to be shipped.
IMPORTANT: Please verify you have all required peripherals available. If CCIT needs to order any extra equipment such as a docking station, be sure to indicate this here.
